In this hospital-based physical therapy role, you will work within a collaborative team, providing exceptional care that makes lasting impacts on patients lives. Youll participate in case management, administer comprehensive therapy modalities in the home setting, and develop individualized plans in partnership with patients, families, physicians, and multidisciplinary teams. Required Qualifications & Experience:
B.S., M.S., or D.P.T. degree in Physical Therapy
Current New York State license in Physical Therapy
BLS certification
At least one year of experience in Long Term Care or Home Care
Recent Home Care experience (within the past year)
Proven Oasis documentation experience
Strong verbal, written, and interpersonal communication skills
Adaptability to work with diverse backgrounds
Ability to perform therapeutic techniques safely, lift, and assist with hands-on care
Professional demeanor, team spirit, and openness to supervision
Key Responsibilities:
Develop, implement, and update physical therapy plans for home-based patients
Serve as liaison between multidisciplinary teams, physicians, and families
Document clinical notes and ensure regulatory compliance
Guide and supervise PTAs and Home Health Aides
Educate patients and caregivers on disease process, self-care, and preventive strategies
Participate in quality improvement initiatives and staff development programs
Foster a supportive, respectful environment with a focus on continuous improvement
