Key qualifications:
- Active California Physical Therapist license
- Previous experience in skilled nursing facilities preferred; mentorship available for those with 0-1 years of experience
- Strong skills in documentation and effective time management
- Flexible, collaborative, and patient-centered approach to care
- Willingness to float between nearby sister facilities as needed
- Schedule flexibilitypreference given to candidates able to work SundayThursday or TuesdaySaturday
Responsibilities include:
- Conducting comprehensive evaluations and implementing individualized treatment plans tailored to each patients needs
- Delivering evidence-based therapeutic interventions to improve resident outcomes
- Collaborating closely with the interdisciplinary care team to ensure cohesive, holistic treatment
- Documenting evaluations, progress notes, and discharge reports using Net Health in accordance with facility standards
