Schedule Options:
- Preference for Sunday Thursday or Tuesday Saturday schedule
Qualifications:
- Active California Physical Therapist (PT) license required
- Skilled nursing facility (SNF) experience preferred; mentorship available for PTs with 01 year of experience
- Excellent documentation and organizational skills
- Collaborative approach to teamwork and an adaptable attitude
- Flexible to float to a sister facility nearby as needed
Key Responsibilities:
- Evaluate, treat, and reassess patients using evidence-based physical therapy practices in a skilled nursing and hospital setting
- Work closely with an interdisciplinary care team to develop individualized treatment plans designed to improve functional independence, strength, and mobility
- Maintain thorough and timely electronic documentation using Net Health, adhering to facility standards
- Support patient progress and satisfaction through regular communication, education, and compassionate clinical care
