Schedule:

• Full-time, contract assignment
• Day shifts

Primary Responsibilities:

• Evaluate and assess referred patients to determine functional limitations and therapy needs
• Develop and implement individualized treatment plans with measurable goals
• Deliver safe, effective, and compassionate physical therapy interventions
• Educate patients and families about ongoing care and at-home strategies to maximize progress
• Collaborate with other members of the healthcare team, including physicians, nurses, and occupational therapists
• Reassess and modify plans as needed to ensure optimal patient outcomes
• Accurately document evaluations, progress, and discharge notes according to facility standards
