Schedule/Hours

Key Responsibilities
- Plan, organize, and direct home care services tailored to each patient's needs

Qualifications
- Valid licensure as a Physical Therapist (PT) in California or eligibility to obtain
- Experience in hospital or home health therapy preferred
- Competence in interdisciplinary collaboration and adaptable care delivery
- Strong organizational and communication skills
