Job Description
Provide expert physical therapy services in a home health setting, supporting patients in Lowndes, Echols, Brooks, Cook, Berrien, and Lanier counties. This contract role offers a Monday through Friday schedule, 8:00 AM to 5:00 PM, with no on-call duties, ideal for therapists seeking a balanced workweek.
Key Responsibilities:
- Conduct comprehensive physical therapy evaluations using established tools and procedures
- Develop and implement individualized treatment plans based on physician referrals
- Regularly reassess patient progress and modify treatment goals as needed
- Communicate patient status and recommendations effectively with medical providers, patients, and families
- Recommend and facilitate access to adaptive equipment to enhance rehabilitation outcomes
- Maintain accurate documentation in compliance with all regulatory and company standards
- Supervise Physical Therapist Assistants and Aides in clinical and patient-related activities
- Participate in staff development and continuing education to maintain clinical expertise
Qualifications:
- Current Physical Therapist license in Georgia or a compact license recognized by the state
- Minimum of 2 years experience in home health physical therapy preferred
- Proficient with HomeCare Homebase software is highly desirable
- Strong communication and organizational skills
- Ability to work independently and manage caseload effectively
