Schedule:

• Monday through Friday
• No on-call requirements

Qualifications:

• Active Physical Therapy license in North Carolina
• Experience with Home Health PT, Oasis, and HCHB highly preferred
• Strong clinical assessment and documentation skills
• Comfort with evaluating and treating patients in home settings across Forsyth, Davidson, Stokes, and Surry Counties

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans based on physician orders
• Reassess progress, adapt treatment goals, and maintain open communication with patients, families, and healthcare teams
• Assist in securing adaptive equipment to maximize patient function and safety
• Maintain timely, accurate documentation in accordance with company and regulatory standards
• Supervise physical therapist assistants and aides in clinical settings
• Participate in ongoing professional development and internal meetings to enhance clinical skills
