This position focuses on planning, organizing, and directing therapy services for patients receiving care at home. Youll independently manage your own caseload while also collaborating closely with interdisciplinary teams to ensure personalized and effective care. By functioning as a key resource for patients, their families, and partner care professionals, youll play an integral role in helping each individual achieve their health and rehabilitation goals.

Desired Experience and Qualifications:

• Active licensure as a Physical Therapist (PT)
• Previous hospital or home care experience strongly preferred
• Demonstrated ability to manage a patient caseload efficiently

• Resourceful approach to problem-solving and patient advocacy
