Role Overview:
You will be performing comprehensive patient evaluations and delivering therapeutic services as prescribed by physicians, ensuring all care adheres to state and professional standards. This position includes supervision and evaluation of Physical Therapy Assistants and Home Health Aides, collaboration with multidisciplinary teams, and active participation in patient care planning and reassessment.

Essential Qualifications:

  • Graduation from an accredited physical therapy program (APTA or equivalent)
  • Current, unencumbered license to practice physical therapy in Oregon (or eligibility)
  • Minimum of 2-3 years of PT experience preferred
  • Demonstrated ability with EMR systems and common computer tools
  • Active CPR certification
  • Reliable transportation, current drivers license, and required liability insurance

Key Responsibilities:

  • Implement therapy interventions in accordance with patient treatment plans
  • Assess and document patient progress and communicate with physicians as needed
  • Develop, modify, and review patient treatment plans
  • Supervise and instruct PTAs and Home Health Aides in delivery of care
  • Educate and consult with patients, families, and healthcare team
  • Participate in clinical meetings, in-service training, and ongoing education
