Provide expert physical therapy services within a dynamic hospital rehabilitation setting, focusing on restoring and enhancing patients physical, social, and mental functions. This role supports interdisciplinary patient care and contributes to patient recovery across multiple hospital units, utilizing advanced equipment and collaborative techniques.

Location: Shafter, CA

Key Responsibilities:

• Plan and conduct individualized treatment segments to improve patient function and meet therapeutic goals
• Collaborate as a co-leader in interdisciplinary patient care teams
• Provide therapy services across various hospital units per orders received, ensuring timely and effective interventions
• Utilize physical therapy equipment including wheelchairs, walkers, and canes to assist patient mobility
• Perform specialized procedures such as video swallow evaluations
• Maintain accurate documentation using the Cerner system
• Participate in scheduled weekend coverage (minimum one weekend shift per month) and be available for optional floating to support other sites or units
• Communicate with intensivists and hospitalists as necessary to coordinate patient care

Qualifications:

• Licensed Physical Therapist with hospital-based rehabilitation experience
• Skilled in using therapy equipment and performing specialized procedures (e.g., video swallows)
• Comfortable working in a fast-paced, multi-unit hospital environment
• Ability to collaborate effectively with interdisciplinary teams
• Proficient with electronic documentation systems, preferably Cerner
• Flexibility to work scheduled weekends and provide occasional float coverage

Schedule & Requirements:

• Day shifts with no night shift required
• Weekend coverage rotation (1 weekend shift per month)
• Call-in availability for unforeseen absences
• Optional floating encouraged to support facility needs
