Key Qualifications:
- Active Physical Therapist (PT) license in California required
- Prior experience in skilled nursing facilities preferred; mentorship provided for candidates with up to one year of experience
- Strong documentation and organizational skills
- Flexible, collaborative attitude, with a willingness to support team needs
- Comfortable with potential floating to a nearby sister facility within walking distance
- Solid communication abilities and dedication to excellent patient outcomes
Preferred Schedule:
- Sunday through Thursday, or
- Tuesday through Saturday
Primary Responsibilities:
- Conduct comprehensive evaluations and develop individualized treatment plans for patients in a skilled nursing setting
- Deliver rehabilitative interventions to improve function, mobility, and quality of life
- Work in partnership with a dedicated interdisciplinary team to ensure holistic, patient-focused care
- Complete thorough and timely documentation of patient progress in Net Health, in accordance with facility standards
