Qualifications:
- Minimum of two years recent experience as an Occupational Therapist
- Active California OT license in good standing
- Current Basic Life Support (BLS) certification
- Proven ability to work independently and manage a varied caseload
- Strong communication, assessment, and documentation skills
- Prior home health experience preferred
Schedule:

Core Responsibilities:
- Provide comprehensive occupational therapy assessments and develop individualized care plans
- Deliver therapies to support patients in regaining independence and functional abilities within their homes
- Collaborate closely with interdisciplinary healthcare teams, including physical therapists and nurses
- Maintain accurate clinical records and timely documentation of patient progress
- Educate patients and caregivers on exercises, equipment use, and home safety for optimal health outcomes
- Monitor patient goals and adjust plans as needed
