Job Description

Qualifications:

  • Minimum of two years recent experience as an Occupational Therapist
  • Active California OT license in good standing
  • Current Basic Life Support (BLS) certification
  • Proven ability to work independently and manage a varied caseload
  • Strong communication, assessment, and documentation skills
  • Prior home health experience preferred

Schedule:

Core Responsibilities:

  • Provide comprehensive occupational therapy assessments and develop individualized care plans
  • Deliver therapies to support patients in regaining independence and functional abilities within their homes
  • Collaborate closely with interdisciplinary healthcare teams, including physical therapists and nurses
  • Maintain accurate clinical records and timely documentation of patient progress
  • Educate patients and caregivers on exercises, equipment use, and home safety for optimal health outcomes
  • Monitor patient goals and adjust plans as needed
