Schedule/Hours:
- Assignments may include weekends
- 36 hours guaranteed (based on meeting weekly productivity expectations; see detailed productivity breakdown)
- Occasional travel outside your main service area required
Qualifications:
- Current California Physical Therapist License
- BLS certification
- Valid drivers license, auto insurance, and vehicle registration
- Up-to-date TB, MMR, Varicella, HepB, TDap (within 10 years), and Flu vaccination (annual)
- Ability to pass a drug screen and physical
- Flexibility to serve various locations within the greater Bay Area and willingness to see COVID+ or Persons Under Investigation as needed
Key Responsibilities:
- Deliver physical therapy services to a diverse patient population
- Complete comprehensive OASIS assessments, evaluations, and documentation
- Maintain weekly productivity standards
- Communicate effectively with patients, caregivers, and clinicians
- Attend to patient visits, documentation, and interprofessional collaboration within designated timelines
