Schedule:

Qualifications:

  • Valid California Physical Therapist license
  • Active BLS certification
  • Valid drivers license, auto insurance, and vehicle registration
  • TB (annual), MMR, Varicella, HepB, TDap (within 10 years), annual flu vaccine
  • Ability to pass drug screening and physical examination
  • Flexibility to see COVID-positive or PUI patients when required

Responsibilities:

  • Conduct comprehensive evaluations, routine visits, discharges, and recertifications using various OASIS formats
  • Collaborate with multidisciplinary teams and families to maximize patient outcomes
  • Provide in-home and hospital-based physical therapy services, including thorough documentation and effective patient communication
