Schedule/Hours:
- Potential need for weekend coverage
- Weekly expectation of 27 points/36 hours for guaranteed assignments
- Flexibility to travel beyond primary territory as needed
Responsibilities:
- Perform evaluations, develop treatment plans, and deliver high-quality physical therapy services in home health settings
- Complete patient documentation, progress notes, and communications with providers
- Meet productivity requirements and coordinate care across multiple service areas
- Provide compassionate support and education to patients and families
- Adhere to safety and compliance standards, including infection control
Qualifications:
- Active California Physical Therapist license
- Current BLS certification
- Valid drivers license, auto insurance, and reliable vehicle
- Up-to-date immunizations (TB, MMR, Varicella, HepB, TDap, Flu)
- Ability to travel throughout assigned areas and adapt to variable schedules
- Experience in home health or hospital settings preferred
