Job Description
Deliver physical therapy services in a home health setting with a focus on timely patient care and thorough documentation. This travel position requires working in Solano County, California, with potential expansion of the territory. The role involves close collaboration with Physical Therapy Assistants and other team members to ensure the highest standard of home health care.
Key Responsibilities:
- Prioritize and complete start of care visits promptly
- Supervise Physical Therapy Assistants effectively
- Submit next-day mandatory reports for start of care visits
- Complete clinical notes and documentation within 48 hours of each visit
- Maintain consistent and timely communication with supervisors, intake staff, and the office
- Stay responsive during normal business hours to support patient care and administrative needs
Qualifications / Desired Experience:
- California Physical Therapy License is required
- Minimum of one year experience in home health physical therapy
- Comfortable using documentation software (Kinnser) and mobile technology such as iPads and communication devices
- Ability to meet weekly productivity targets and manage caseload efficiently
Schedule:
- 40 hours per week, working Saturday through Wednesday
- Three full days of training provided (one in-house, two in the field)
