Schedule Options:
- Preference for Sunday through Thursday or Tuesday through Saturday
- Contract role; may include float assignments to a nearby sister facility within walking distance
Desired Qualifications:
- Current, active Physical Therapist license in California
- Experience in skilled nursing facilities is preferred; mentorship is available for those with 0-1 years of experience
- Proficient documentation and strong time-management skills
- Ability to adapt and work collaboratively within an interdisciplinary care environment
- Flexible approach to shifting patient needs and facility assignments
Key Responsibilities:
- Evaluate and develop individualized treatment plans for patients in a skilled nursing facility setting
- Deliver quality therapy services aimed at improving patients functional abilities, mobility, and overall independence
- Partner closely with an interdisciplinary healthcare team to optimize patient care and outcomes
- Complete timely and accurate documentation in Net Health, following facility standards and regulatory requirements
- Demonstrate a compassionate, patient-centered approach to therapy services
