Job Description

Provide expert rehabilitative care as a Physical Therapist in a skilled nursing facility setting focused on improving patient mobility and independence. This contract opportunity is based in Roseville, CA, within a supportive inpatient environment dedicated to long-term care and short-term rehabilitation.

Key Responsibilities: Conduct comprehensive assessments of patients physical capabilities, limitations, and pain to inform treatment. Develop personalized physical therapy plans aligned with patient goals and clinical findings. Deliver interventions targeting strength, coordination, balance, and functional mobility enhancement. Collaborate closely with interdisciplinary teams including physicians, nurses, occupational and speech therapists, and families. Monitor progress, adjust therapies, and accurately document care in compliance with regulatory standards. Educate patients and caregivers on safe mobility practices, home exercise programs, and appropriate equipment use. Participate actively in care planning and team conferences. Supervise physical therapist assistants and aides as required. Maintain safety and cleanliness of the therapy environment.

Qualifications and Desired Experience: Graduate of an accredited Physical Therapy program with current state licensure. CPR certification preferred or as required by facility guidelines. Prior experience in skilled nursing or geriatric rehabilitation settings highly valued. Strong communication skills and proficiency in thorough documentation. Ability to work independently while engaging effectively within multidisciplinary teams.
