Hours/Schedule:
- Travel position with home visits and periodic office meetings
- Schedule flexibility to explore the region during downtime
Key Qualifications:
- Graduation from an accredited physical therapy program (APTA or equivalent)
- Unencumbered, active Oregon physical therapy license (or eligibility)
- 2-3 years experience in physical therapy preferred
- Current CPR certification
- Computer proficiency (navigating EMR, emails, and online HR tools)
- Reliable transportation, valid drivers license, and required liability insurance
Core Responsibilities:
- Evaluate patients and deliver therapeutic services as prescribed by physicians
- Develop, update, and manage patients treatment plans
- Supervise and assess Physical Therapy Assistants and Home Health Aides as assigned
- Observe, document, and communicate patient condition and progress
- Advise family members, caregivers, and agency staff on therapy plans
- Participate in in-service education and quality improvement initiatives
- Conduct home environment evaluations and provide recommendations for care
- Ensure compliance with all applicable state and agency standards
