Hours/Schedule:

  • Travel position with home visits and periodic office meetings
  • Schedule flexibility to explore the region during downtime

Key Qualifications:

  • Graduation from an accredited physical therapy program (APTA or equivalent)
  • Unencumbered, active Oregon physical therapy license (or eligibility)
  • 2-3 years experience in physical therapy preferred
  • Current CPR certification
  • Computer proficiency (navigating EMR, emails, and online HR tools)
  • Reliable transportation, valid drivers license, and required liability insurance

Core Responsibilities:

  • Evaluate patients and deliver therapeutic services as prescribed by physicians
  • Develop, update, and manage patients treatment plans
  • Supervise and assess Physical Therapy Assistants and Home Health Aides as assigned
  • Observe, document, and communicate patient condition and progress
  • Advise family members, caregivers, and agency staff on therapy plans
  • Participate in in-service education and quality improvement initiatives
  • Conduct home environment evaluations and provide recommendations for care
  • Ensure compliance with all applicable state and agency standards
