Essential Qualifications:
- Graduate of an accredited Physical Therapy program
- Active state license to practice as a Physical Therapist (PT)
- CPR certification (required or preferred, as determined by the facility)
- Previous experience in a skilled nursing or geriatric rehabilitation setting preferred
- Strong verbal, written, and interpersonal communication abilities
- Proven ability to work collaboratively within a multidisciplinary care environment
Schedule/Hours:
- Contract position; details provided throughout the interview process
Key Responsibilities:
- Conduct comprehensive patient evaluations to assess physical abilities, pain level, and restrictions
- Develop and implement individualized treatment plans aimed at improving mobility, strength, balance, and function
- Deliver evidence-based physical therapy interventions, adapting plans as needed based on ongoing assessments
- Maintain accurate and timely clinical documentation per regulatory standards
- Educate patients and caregivers on safe movement, at-home exercises, and appropriate use of assistive equipment
- Participate in interdisciplinary meetings to review, coordinate, and optimize patient care
- Ensure compliance with Medicare/Medicaid and facility-specific guidelines
- Supervise and provide direction to physical therapist assistants and aides as needed
- Foster a clean and safe therapeutic environment
