Essential Qualifications:

• Graduate of an accredited Physical Therapy program
• Active state license to practice as a Physical Therapist (PT)
• CPR certification (required or preferred, as determined by the facility)
• Previous experience in a skilled nursing or geriatric rehabilitation setting preferred
• Strong verbal, written, and interpersonal communication abilities
• Proven ability to work collaboratively within a multidisciplinary care environment

Schedule/Hours:

Key Responsibilities:

• Conduct comprehensive patient evaluations to assess physical abilities, pain level, and restrictions
• Develop and implement individualized treatment plans aimed at improving mobility, strength, balance, and function
• Deliver evidence-based physical therapy interventions, adapting plans as needed based on ongoing assessments
• Maintain accurate and timely clinical documentation per regulatory standards
• Educate patients and caregivers on safe movement, at-home exercises, and appropriate use of assistive equipment
• Participate in interdisciplinary meetings to review, coordinate, and optimize patient care
• Ensure compliance with Medicare/Medicaid and facility-specific guidelines
• Supervise and provide direction to physical therapist assistants and aides as needed
• Foster a clean and safe therapeutic environment
