Schedule/Hours:
- Monday to Friday, 8 am 5 pm
- No on-call responsibilities
Desired Qualifications:
- Current West Virginia Physical Therapy license
- Previous hospital-based physical therapy experience required
- Proficiency with HomeCare HomeBase (HCHB) software
- Demonstrated ability to supervise PT Assistants and Aides
- Valid drivers license; ability to travel within assigned counties
Key Responsibilities:
- Perform comprehensive evaluations and develop individualized treatment plans based on physician orders and referrals
- Reassess and adjust therapy plans to achieve patient goals and maximize independence
- Coordinate care and progress updates with the multidisciplinary team, medical providers, patients, and their families
- Document evaluations, treatments, and billing in compliance with all relevant policies and regulations
- Recommend adaptive equipment and integrate it into patient rehab programs
- Supervise and mentor PT Assistants and Aides
- Stay current with internal staff development, required continuing education, and clinical best practices
