Core Responsibilities:

• Manage comprehensive patient care plans in-home and hospital settings
• Conduct assessments, develop and revise plans of care, and coordinate discharge planning
• Deliver quality PT services, including gait training, ADL assistance, and equipment education
• Supervise PT aides and home health team to ensure compliance with therapy plans
• Maintain accurate, regulatory-compliant clinical documentation
• Participate in collaborative care meetings and ongoing professional development initiatives
• Engage in continuous quality improvement by responding to feedback and fostering patient-centered solutions

Required Qualifications:

• B.S., M.S., or D.P.T. in Physical Therapy
• Active New York State PT license
• BLS certification
• At least one year of experience in long-term or home care physical therapy; recent home care experience required
• Oasis proficiency essential
• Strong interpersonal, communication, and patient instruction skills
• Ability to provide hands-on care and collaborate across socio-cultural backgrounds
