Core Responsibilities:
- Manage comprehensive patient care plans in-home and hospital settings
- Conduct assessments, develop and revise plans of care, and coordinate discharge planning
- Deliver quality PT services, including gait training, ADL assistance, and equipment education
- Supervise PT aides and home health team to ensure compliance with therapy plans
- Maintain accurate, regulatory-compliant clinical documentation
- Participate in collaborative care meetings and ongoing professional development initiatives
- Engage in continuous quality improvement by responding to feedback and fostering patient-centered solutions
Required Qualifications:
- B.S., M.S., or D.P.T. in Physical Therapy
- Active New York State PT license
- BLS certification
- At least one year of experience in long-term or home care physical therapy; recent home care experience required
- Oasis proficiency essential
- Strong interpersonal, communication, and patient instruction skills
- Ability to provide hands-on care and collaborate across socio-cultural backgrounds
