Qualifications:

• Bachelors, Masters, or Doctorate in Physical Therapy
• Current New York State PT license
• BLS certification
• At least one year of recent PT experience in a Long Term Care or Home Care/Home Health setting
• Oasis experience required
• Demonstrated ability to assist and lift patients safely
• Excellent written, verbal, and interpersonal skills
• Comfortable working with diverse populations
• Ability to accept and utilize supervision effectively

Key Responsibilities:

• Provide comprehensive physical therapy in a home setting, working closely with patients, families, physicians, and interdisciplinary teams
• Develop, implement, and revise individualized care plans and discharge plans
• Document thoroughly to ensure compliance and continuity of care
• Educate patients, families, and team members on therapy goals and disease management
• Supervise PTAs and coordinate with Home Health Aides
• Support ongoing quality initiatives and participate in continuous learning opportunities
• Maintain strong relationships within the local healthcare community
