Qualifications:
- A B.S., M.S., or D.P.T. in Physical Therapy
- Valid New York State Physical Therapy license
- BLS certification
- Minimum one year of recent experience in Long Term Care/Home Care environments
- Demonstrated recent Home Care experience (within the past year)
- Strong verbal and written communication skills
- Ability to work with diverse populations and effectively deliver hands-on care
- Experience with Oasis documentation required
Primary Responsibilities:
- Provide case management and direct patient care in a home health setting
- Develop, revise, and implement individualized care plans in collaboration with physicians, families, and interdisciplinary teams
- Supervise and mentor PTAs and para-professional staff
- Conduct patient assessments, maintain accurate clinical records, and ensure compliance with regulatory standards
- Participate in patient and team conferences to coordinate high-quality care
