Qualifications:

  • A B.S., M.S., or D.P.T. in Physical Therapy
  • Valid New York State Physical Therapy license
  • BLS certification
  • Minimum one year of recent experience in Long Term Care/Home Care environments
  • Demonstrated recent Home Care experience (within the past year)
  • Strong verbal and written communication skills
  • Ability to work with diverse populations and effectively deliver hands-on care
  • Experience with Oasis documentation required

Primary Responsibilities:

  • Provide case management and direct patient care in a home health setting
  • Develop, revise, and implement individualized care plans in collaboration with physicians, families, and interdisciplinary teams
  • Supervise and mentor PTAs and para-professional staff
  • Conduct patient assessments, maintain accurate clinical records, and ensure compliance with regulatory standards
  • Participate in patient and team conferences to coordinate high-quality care
  • Engage in quality improvement initiatives and professional development opportunities
