Schedule/Hours:
- Contract position
- Coverage includes both Acute Care and Extended Living Center settings
What Were Looking For:
- Current license as a Physical Therapist
- Previous experience working in hospital-based therapy, especially acute and long-term care
- Strong assessment and treatment planning skills
- Excellent communication and collaboration abilities with patients, families, and interdisciplinary teams
- Commitment to delivering personalized patient care and fostering independence
- Familiarity with electronic medical record systems preferred
Your Responsibilities:
- Evaluate patients and create individualized physical therapy treatment plans tailored to acute and long-term care needs
- Collaborate with nurses, physicians, and other care providers to ensure comprehensive care
- Educate patients and families regarding treatment progress and home/inpatient exercise programs
- Help patients maximize their functional potential and mobility for a safe transition home or continued care
- Document all care clearly and effectively in the electronic health record
- Contribute actively to a positive and supportive hospital environment
