Schedule/Hours:
- Expect a commitment of up to 36 hours weekly, with the possibility of weekend coverage.
- Flexibility to travel outside specified areas may be requested to meet patient care needs.
Preferred Qualifications:
- California PT license (active)
- BLS certification
- Valid drivers license, auto registration, and auto insurance
- Up-to-date vaccinations: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
- Successful drug screen and physical
Key Responsibilities:
- Deliver comprehensive, patient-focused physical therapy services in home health/hospital settings
- Complete a defined number of patient visits per week, including OASIS assessments, recertifications, evaluations, discharges, and routine care
- Maintain accurate and timely documentation per visit
- Collaborate with a wider care team and communicate effectively with patients and providers
- Adapt readily to schedule changes and area travel requirements as needed
