Schedule/Hours:
- 36-hour guarantee possible (based on meeting visit expectations)
- Weekend coverage may be required
- Flexibility for occasional travel outside primary areas
Key Qualifications:
- Current California Physical Therapy (PT) license
- BLS certification
- Valid driver's license, auto registration, and insurance
- Up-to-date TB, MMR, Varicella, HepB, Tdap (within 10 years), annual flu vaccine, drug screening, and physical exam
Primary Responsibilities:
- Deliver evidence-based therapy in both home health and hospital settings
- Complete and document OASIS and other visit types per clinical and administrative standards
- Manage a flexible caseload, including working with Covid+ and Covid PUI patients
- Travel within assigned areas and adjust to caseload needs
- Maintain collaborative communication with patients, physicians, and care teams
