Join a dynamic home health therapy position serving Solano County, California, as a Physical Therapist specializing in home health care. This travel role offers a structured schedule and the opportunity to deliver high-quality patient care in a community setting.

Responsibilities:

• Prioritize and complete start of care visits promptly
• Supervise Physical Therapy Assistants effectively
• Report start of care visits by the next day
• Complete clinical notes and documentation within 48 hours of each visit
• Maintain timely and clear communication with supervisors, intake coordinators, and office staff
• Be responsive during normal business hours to support patient and team needs
• Manage patient caseloads averaging 40 hours per week, with visits designed to meet productivity expectations

Qualifications:

• Valid California Physical Therapist license
• Minimum of one year experience in home health physical therapy
• Familiarity with Kinnser documentation system preferred
• Ability to utilize technology such as iPads and communication devices effectively
• Reliable transportation for patient visits within Solano County; willingness to expand territory as needed

Schedule:

• 40 hours per week
• Saturday through Wednesday
