Schedule/Hours:
- Full-time, 5 x 8-hour shifts
- Rotating every other Saturday
- Weekend, holiday, floating, and on-call duties as determined by department needs
Qualifications:
- Graduate of an APTA-accredited Physical Therapy program
- Active PT license and BLS certification required
- Hospital-based and/or home health PT experience preferred
- Adaptability and proficiency in independently managing varied caseloads
Primary Responsibilities:
- Conduct detailed patient evaluations, identifying functional strengths and barriers
- Develop and implement tailored treatment plans, addressing patients holistic needs
- Maintain thorough documentation aligned with regulatory and professional standards
- Effectively communicate with caregivers and interdisciplinary teams to optimize care
- Flexibly respond to changing schedules, caseloads, and departmental requirements
