Working in this travel role allows you to collaborate with multidisciplinary teams, encounter a variety of patient needs, and learn from new leadership perspectives. Schedule/Hours:
- Travel/field-based
- Flexibility required for home visits and periodic office meetings
Key Qualifications:
- Graduation from an accredited physical therapy program (APTA/AMA approved)
- Unencumbered state licensure in SC (or ability to obtain)
- 2-3 years of professional PT experience preferred
- Current CPR certification
- Comfortable with EMR systems and basic computer skills
- Reliable transportation, valid drivers license, and agency-required liability insurance
Responsibilities:
- Evaluate patients and implement therapeutic interventions per physician orders
- Develop and update individualized plans of care
- Supervise and evaluate PTAs and Home Health Aides, ensuring safe, effective treatment
- Maintain thorough documentation and communication with physicians
- Educate patients, families, and other agency staff in therapy goals and plans
- Recommend adaptive equipment and environmental modifications as needed
- Participate in in-service education, quality improvement initiatives, and care team collaboration
- Conduct regular home visits and periodic office meetings
