Schedule/Hours:

  • Travel assignment with flexibility for field and home visits

Key Qualifications:

  • Graduation from a physical therapy program accredited by the American Physical Therapy Association or equivalent
  • Active, unencumbered PT license for NY (or eligibility to obtain)
  • 2-3 years of experience in physical therapy preferred
  • Current CPR certification
  • Proficient with basic computer tasks and patient documentation
  • Reliable transportation, valid drivers license, and required liability insurance

Role Highlights and Responsibilities:

  • Perform patient evaluations and deliver therapeutic services per physician orders
  • Develop, update, and report on individualized plans of care
  • Supervise and assess Physical Therapy Assistants and Home Health Aides as assigned
  • Advise and instruct family/caregivers and collaborate with other healthcare personnel
  • Document clinical progress and maintain accurate health records
  • Participate in in-service education and contribute to quality improvement initiatives
  • Evaluate home environments and recommend adaptive solutions as needed

Compensation for this position ranges from $28.06 to $50.00. Eligible candidates may also receive healthcare benefits, housing, and meals/incidentals. Full details of our healthcare benefits plan can be found here. This posting is open for 60 days after the posting date.
