In this role, you will deliver therapeutic services within hospital settings, ensuring optimal outcomes for patients as directed by physicians. You will also supervise and evaluate physical therapy assistants and home health aides, contributing to an impactful, team-focused environment.

Schedule/Hours:

  • Flexible scheduling based on patient needs and home visits
  • Periodic in-office meetings required

Key Qualifications:

  • Graduate of an accredited physical therapy program (approved by APTA or the relevant medical authority)
  • Current, active, and unencumbered physical therapy license in Oregon (or eligible for state licensure)
  • Minimum of 2-3 years experience preferred
  • Valid drivers license, reliable transportation, and required liability insurance
  • Current CPR certification
  • Competency in using computers and navigating online systems

Responsibilities:

  • Perform patient evaluations, develop and update individualized care plans
  • Provide direct therapeutic services under physician orders
  • Supervise and mentor PTAs and home health aides
  • Maintain clear, timely documentation and communicate effectively with the healthcare team
  • Educate patients and families regarding treatment programs
  • Evaluate patient environments and recommend modifications for safety and independence
  • Attend in-service education and collaborate on discharge planning
