Job Description

In this vital role within a reputable skilled nursing facility, you will deliver high-quality therapeutic care to patients aiming to regain mobility and functional independence. You will be responsible for assessing patient needs, developing individualized therapy plans, and executing tailored interventions to support patient rehabilitation. The position offers a supportive environment, opportunities for mentorship, and requires adaptability, as floatation to a nearby sister facility may occasionally be needed.

Schedule Options:

  • Sunday through Thursday
  • Tuesday through Saturday

Qualifications:

  • Active Physical Therapist (PT) license in California
  • Skilled nursing facility experience preferred (mentorship available for clinicians with 0-1 years experience)
  • Proficiency in documentation and excellent time management
  • Flexible, collaborative approach to patient care
  • Comfortable with occasional floatation within a walkable distance

Key Responsibilities:

  • Conduct evaluations of patients in the skilled nursing facility setting
  • Develop and implement individualized treatment plans focused on restoring mobility and strength
  • Deliver therapeutic interventions that align with each patients needs and care goals
  • Maintain thorough documentation in Net Health per facility requirements
  • Partner with an interdisciplinary team to ensure optimal patient outcomes
