The position focuses on improving residents' mobility, strength, and independence, with support for both experienced professionals and those newer to skilled nursing settings.

Schedule Options:

• Sunday through Thursday or
• Tuesday through Saturday

Qualifications:

• Current and active Physical Therapist license in California
• Previous experience in a skilled nursing facility strongly preferred; mentorship offered for those with 01 year of experience
• Excellent documentation and time-management abilities
• Flexible approach with a collaborative mindset
• Willingness to float to a nearby sister facility within walking distance, as needed

Primary Responsibilities:

• Conduct thorough patient assessments and develop individualized care plans
• Deliver targeted therapy to improve patient mobility, strength, and daily living skills
• Collaborate closely with an interdisciplinary care team to advance patient outcomes
• Complete timely and accurate documentation using Net Health in compliance with facility standards
• Maintain compassionate, patient-centered care at all times
