The position focuses on improving residents' mobility, strength, and independence, with support for both experienced professionals and those newer to skilled nursing settings.
Schedule Options:
- Sunday through Thursday or
- Tuesday through Saturday
Qualifications:
- Current and active Physical Therapist license in California
- Previous experience in a skilled nursing facility strongly preferred; mentorship offered for those with 01 year of experience
- Excellent documentation and time-management abilities
- Flexible approach with a collaborative mindset
- Willingness to float to a nearby sister facility within walking distance, as needed
Primary Responsibilities:
- Conduct thorough patient assessments and develop individualized care plans
- Deliver targeted therapy to improve patient mobility, strength, and daily living skills
- Collaborate closely with an interdisciplinary care team to advance patient outcomes
- Complete timely and accurate documentation using Net Health in compliance with facility standards
- Maintain compassionate, patient-centered care at all times
