Job Description

Take on a critical role providing exceptional physical therapy services in a dynamic home health setting supporting adult patients with chronic diseases and orthopedic conditions. This opportunity is structured as a contract position with a focus on flexibility and strong team collaboration. Therapists in this position will travel throughout a variety of service areas, conducting an average of six patient visits each day, with daily driving averaging between 60 to 80 miles. Mileage is reimbursed weekly, and a company cell phone is provided for ease of documentation and communication.

Schedule Details:

  • No weekend rotation
  • No on-call requirement
  • No major holidays
  • Schedule cycle is every 13 weeks
  • Time-off requests are subject to manager approval

Required Qualifications:

  • At least 1 year of home health experience
  • Current state licensure as a Physical Therapist (PT)
  • BLS certification
  • Drivers license and proof of insurance
  • Open to first-time travelers with seasoned home health backgrounds

Key Responsibilities:

  • Deliver skilled physical therapy services to adult home health patients
  • Assess, plan, and implement individualized treatment interventions
  • Schedule daily patient visits in advance
  • Maintain accurate documentation using provided technology
  • Collaborate closely with departmental leadership, the clinical team, and supervisors for ongoing support
  • Meet a minimum of 30 visit equivalents per week
  • Participate actively in a supportive, team-based departmental culture
