Key Responsibilities:
- Deliver evaluative and therapeutic PT services in compliance with physician orders and established protocols
- Formulate and implement individualized plans of care with functional, measurable goals
- Consult with interdisciplinary teams to optimize patient outcomes and ensure effective discharge planning
- Provide guidance on and instruct use of assistive devices to residents and caregivers
- Diligently document all interventions in the electronic medical record system
- Supervise and mentor PT assistants and new staff, promoting high quality standards
- Lead in-service education sessions to support team and organizational knowledge
- Ensure compliance with Medicare, Medicaid, and insurance requirements regarding documentation and reimbursement
- Maintain clean, safe, and effective therapy environments
Qualifications:
- Bachelors degree required (Masters/Doctorate preferred)
- Current PT license in Massachusetts
- Previous experience in long-term care or skilled nursing greatly valued
- Basic Life Support (BLS) certification preferred
- Outstanding interpersonal, communication, and documentation skills
- Proficient with Microsoft Office and electronic health records
- Able to comprehend, communicate, and document in English
