Schedule & Location
- Monday through Friday, 8 am to 5 pm
- Multiple counties covered: Butler, Crenshaw, Lowndes, Covington
Key Qualifications
- Active Physical Therapist license
- Previous Home Health experience required
- HCHB (Homecare Homebase) experience preferred but not required
- Valid drivers license and ability to travel between locations (mileage reimbursement provided)
Primary Responsibilities
- Evaluate and treat patients according to physician orders/referrals
- Establish treatment goals and individualized plans to maximize patient independence
- Reassess and modify treatment plans based on patient progress
- Communicate effectively with the treatment team, physicians, patients, and families
- Recommend adaptive equipment as needed for optimal patient outcomes
- Maintain documentation in accordance with state/federal requirements
- Ensure compliance with organizational and professional standards
- Supervise Physical Therapist Assistants and Aides
- Participate in ongoing training and professional development
