Schedule & Location

  • Monday through Friday, 8 am to 5 pm
  • Multiple counties covered: Butler, Crenshaw, Lowndes, Covington

Key Qualifications

  • Active Physical Therapist license
  • Previous Home Health experience required
  • HCHB (Homecare Homebase) experience preferred but not required
  • Valid drivers license and ability to travel between locations (mileage reimbursement provided)

Primary Responsibilities

  • Evaluate and treat patients according to physician orders/referrals
  • Establish treatment goals and individualized plans to maximize patient independence
  • Reassess and modify treatment plans based on patient progress
  • Communicate effectively with the treatment team, physicians, patients, and families
  • Recommend adaptive equipment as needed for optimal patient outcomes
  • Maintain documentation in accordance with state/federal requirements
  • Ensure compliance with organizational and professional standards
  • Supervise Physical Therapist Assistants and Aides
  • Participate in ongoing training and professional development
