Provide skilled physical therapy services to patients in a home health setting, ensuring treatment aligns with physicians' orders and individualized plans of care. This contract opportunity based in Marlborough, MA involves patient evaluations, therapy delivery, and oversight of physical therapy assistants and home health aides, promoting optimal recovery and functional improvement.
Key Responsibilities:
- Perform patient evaluations and administer therapeutic services as prescribed by the attending physician.
- Supervise and evaluate physical therapy assistants and home health aides assigned to your care.
- Collaborate with physicians to develop and update individualized treatment plans.
- Document patient progress and communicate findings to the healthcare team.
- Educate and support patients' families and caregivers in therapy programs.
- Assess home environments and recommend modifications to support patient recovery.
- Participate in ongoing educational programs to maintain clinical competency.
Desired Qualifications:
- Successful completion of an accredited physical therapy program.
- Current and unrestricted state license to practice physical therapy in Massachusetts.
- Minimum of 2-3 years of physical therapy experience preferred.
- Valid CPR certification.
- Proficient with computer systems including email, electronic health records, and online training platforms.
- Reliable transportation and valid driver's license for home visits.
