Qualifications:
- Valid California Physical Therapist license
- Prior experience in skilled nursing facilities preferred; mentorship provided for candidates with 01 year of experience
- Excellent clinical documentation and time management abilities
- Strong teamwork and adaptability, with openness to learning and collaborating
- Willingness to work at a nearby facility within walking distance if needed
Primary Responsibilities:
- Conduct patient assessments and develop individualized treatment plans
- Deliver evidence-based physical therapy interventions to improve functional outcomes
- Work closely with the interdisciplinary team, including nurses and occupational/speech therapists, to coordinate care
- Complete and maintain clear, accurate documentation using Net Health EMR per facility protocols
