Qualifications:

• Bachelors degree (required); Masters or Doctorate (preferred)
• Active Physical Therapist license in Massachusetts
• Basic Life Support (BLS) certification (preferred)
• Experience in long-term care or skilled nursing preferred

• Strong team collaboration and time management skills

Key Responsibilities:

• Conduct evaluations and develop individualized therapy plans in collaboration with residents and care teams
• Consult with physicians and participate in team meetings on care and discharge planning
• Document all clinical activities clearly in the Electronic Medical Record
• Educate residents, families, and staff regarding therapy plans and assistive equipment
• Mentor new team members and supervise assistant-level staff
• Ensure adherence to clinical, regulatory, and infection control guidelines
