Schedule/Hours:

  • Travel position; flexible scheduling aligned with patient needs.

Qualifications:

  • Bachelors degree in Physical Therapy (Masters or Doctorate preferred)
  • Active PT license for Massachusetts
  • BLS (Basic Life Support) certification preferred
  • Prior experience in long-term care or skilled nursing facility is a plus
  • Excellent communication, collaboration, and time management skills
  • Proficient in Microsoft Office
  • Ability to lift up to 50 pounds and perform job-related physical tasks

Key Responsibilities:

  • Evaluate and implement personalized therapeutic plans under physician guidance
  • Partner with residents and caregivers to maximize independence and daily function
  • Document all assessments, interventions, and care progress in EMR
  • Provide education to residents, caregivers, and staff
  • Collaborate with interdisciplinary teams for holistic resident care
  • Mentor new staff and guide assistant-level team members as appropriate
  • Maintain compliance with state and federal regulations
