Qualifications:
- Bachelors degree (required); Masters or Doctorate (preferred)
- Active Physical Therapist license in Massachusetts
- Basic Life Support (BLS) certification (preferred)
- Experience in long-term care or skilled nursing preferred

- Strong team collaboration and time management skills
Key Responsibilities:
- Conduct evaluations and develop individualized therapy plans in collaboration with residents and care teams
- Consult with physicians and participate in team meetings on care and discharge planning
- Document all clinical activities clearly in the Electronic Medical Record
- Educate residents, families, and staff regarding therapy plans and assistive equipment
- Mentor new team members and supervise assistant-level staff
- Ensure adherence to clinical, regulatory, and infection control guidelines
