Job Description

Deliver skilled physical therapy services in a home health setting, performing patient evaluations and providing treatment according to physician orders. This contract position involves supervising Physical Therapy Assistants and Home Health Aides as assigned, ensuring high-quality patient care aligned with state-specific regulations.

Key Responsibilities:

  • Conduct thorough patient assessments and develop individualized treatment plans.
  • Administer therapeutic services in compliance with physicians directives.
  • Supervise and evaluate Physical Therapy Assistants and Home Health Aides.
  • Monitor patient progress, documenting and reporting findings accurately.
  • Collaborate with physicians, families, and agency personnel to optimize therapy outcomes.
  • Evaluate home environments and recommend safe and effective adjustments.
  • Participate in ongoing education and service improvement initiatives.
  • Adhere to state licensing and practice regulations, including regular in-person assessments.

Qualifications and Experience:

  • Graduation from an accredited physical therapy program recognized by relevant professional bodies.
  • Current, valid physical therapy license for the state of assignment.
  • Minimum of 2-3 years experience in physical therapy preferred.
  • CPR certification up to date.
  • Proficiency with computers, including email, internet navigation, and electronic health records.
  • Reliable transportation, valid drivers license, and agency-required liability insurance.
