Schedule/Hours:

Responsibilities:

• Conduct comprehensive assessments of patients physical status, mobility limitations, and pain levels
• Create and implement individualized treatment plans tailored to patient goals and clinical findings
• Apply proven therapeutic interventions aimed at improving strength, coordination, balance, and daily function
• Collaborate closely with physicians, nursing staff, and allied health professionals to optimize patient care
• Monitor and document patient progress, modifying interventions as appropriate
• Educate patients and their caregivers on injury prevention, safe mobility strategies, and home exercise programs
• Ensure regulatory compliance including accurate Medicare/Medicaid documentation
• Participate in interdisciplinary care planning meetings
• Supervise physical therapist assistants and therapy aides as needed
• Maintain a safe, clean, and motivating treatment environment

Qualifications:

• Degree from an accredited Physical Therapy program
• Current state licensure as a Physical Therapist
• CPR certification (required or preferred per facility guidelines)
• Experience in skilled nursing or geriatric rehabilitation settings highly valued
• Strong interpersonal, teamwork, and documentation skills
• Ability to work independently and collaborate effectively within an interdisciplinary team
