Schedule/Hours:

Responsibilities:
- Conduct comprehensive assessments of patients physical status, mobility limitations, and pain levels
- Create and implement individualized treatment plans tailored to patient goals and clinical findings
- Apply proven therapeutic interventions aimed at improving strength, coordination, balance, and daily function
- Collaborate closely with physicians, nursing staff, and allied health professionals to optimize patient care
- Monitor and document patient progress, modifying interventions as appropriate
- Educate patients and their caregivers on injury prevention, safe mobility strategies, and home exercise programs
- Ensure regulatory compliance including accurate Medicare/Medicaid documentation
- Participate in interdisciplinary care planning meetings
- Supervise physical therapist assistants and therapy aides as needed
- Maintain a safe, clean, and motivating treatment environment
Qualifications:
- Degree from an accredited Physical Therapy program
- Current state licensure as a Physical Therapist
- CPR certification (required or preferred per facility guidelines)
- Experience in skilled nursing or geriatric rehabilitation settings highly valued
- Strong interpersonal, teamwork, and documentation skills
- Ability to work independently and collaborate effectively within an interdisciplinary team
