Schedule/Hours:

  • Monday thru Friday
  • No on-call requirement

Key Responsibilities:

  • Perform comprehensive physical therapy evaluations and treatments based on physician referrals.
  • Use appropriate assessment tools and procedures in line with best practices.
  • Review patient records thoroughly to identify precautions and inform therapy approaches.
  • Establish measurable goals and create individualized treatment plans to maximize patient independence and function.
  • Reassess and update treatment goals as needed, ensuring optimal patient outcomes.
  • Communicate regularly with the treatment team, families, and medical providers regarding patient progress.
  • Recommend adaptive equipment to support rehabilitation and help source suitable devices for patients.
  • Ensure accurate and timely documentation of evaluations, treatments, and billing in compliance with regulations.
  • Supervise Physical Therapist Assistants and Aides, ensuring quality direct patient care.
  • Participate in ongoing staff development, meetings, and required continuing education.

Qualifications & Desired Experience:

  • Active Physical Therapy license in North Carolina.
  • Prior experience in Home Health PT, with proficiency in OASIS and HCHB.
  • Strong clinical evaluation and treatment planning skills.
  • Exceptional communication and collaboration abilities.
  • Commitment to ethical practice and regulatory compliance.
