Job Description
This role involves planning and conducting treatment programs aimed at restoring and enhancing patients physical, social, and mental functions. You will work collaboratively as a co-leader within an interdisciplinary team to provide comprehensive care tailored to patient needs.
Key Responsibilities:
- Develop and implement individualized therapy plans targeting physical rehabilitation
- Collaborate with healthcare professionals to coordinate patient care
- Monitor patient progress and adjust treatment strategies accordingly
- Educate patients and families on therapy goals and home exercise programs
- Maintain detailed and accurate documentation in compliance with department standards
Desired Experience & Qualifications:
- Valid Physical Therapy license
- Experience in hospital-based physical therapy preferred
- Strong communication and teamwork skills
- Ability to lead and contribute effectively in a multidisciplinary team
- Commitment to patient-centered care and outcomes
