Key Responsibilities:
- Deliver physical therapy services to residents, maximizing their functional independence
- Conduct comprehensive evaluations and create individualized plans of care in compliance with established protocols
- Incorporate resident and caregiver input into therapeutic objectives
- Consult with physicians and interdisciplinary team members on care and discharge plans
- Assess treatment outcomes, adjust plans as needed, and document all care in the Electronic Medical Record (EMR)
- Recommend and provide training on assistive devices, and educate residents and caregivers
- Collaborate with and supervise Physical Therapist Assistants and other therapy staff
- Comply with Medicare, Medicaid, and other payer requirements, ensuring thorough and clear documentation
- Mentor new staff during orientation and support ongoing department initiatives
- Participate in departmental in-service education, maintain professional competencies, and contribute to a clean, safe environment
Required Experience and Qualifications:
- Bachelors degree required; Masters or Doctorate in Physical Therapy preferred
- Current, active PT license in Massachusetts
- Basic Life Support (BLS) certification preferred
- Previous experience in long-term or skilled nursing care environments is advantageous
- Strong communication, teamwork, and time-management skills
- Proficiency in Microsoft Office and medical documentation systems
- Ability to lift up to 50 lbs (with a team member if over 50 lbs) and perform regular physical activities integral to the role
