Key Responsibilities:

• Deliver physical therapy services to residents, maximizing their functional independence
• Conduct comprehensive evaluations and create individualized plans of care in compliance with established protocols
• Incorporate resident and caregiver input into therapeutic objectives
• Consult with physicians and interdisciplinary team members on care and discharge plans
• Assess treatment outcomes, adjust plans as needed, and document all care in the Electronic Medical Record (EMR)
• Recommend and provide training on assistive devices, and educate residents and caregivers
• Collaborate with and supervise Physical Therapist Assistants and other therapy staff
• Comply with Medicare, Medicaid, and other payer requirements, ensuring thorough and clear documentation
• Mentor new staff during orientation and support ongoing department initiatives
• Participate in departmental in-service education, maintain professional competencies, and contribute to a clean, safe environment

Required Experience and Qualifications:

• Bachelors degree required; Masters or Doctorate in Physical Therapy preferred
• Current, active PT license in Massachusetts
• Basic Life Support (BLS) certification preferred
• Previous experience in long-term or skilled nursing care environments is advantageous
• Strong communication, teamwork, and time-management skills
• Proficiency in Microsoft Office and medical documentation systems
• Ability to lift up to 50 lbs (with a team member if over 50 lbs) and perform regular physical activities integral to the role
