Job Description

Make a meaningful impact on patient recovery and independence by providing comprehensive physical therapy services in a hospital-based role. This position focuses on the delivery of skilled PT care across several counties and supports patients from initial evaluation to discharge.

Schedule:

  • Monday to Friday, 8:00am 5:00pm
  • No on-call requirements

Key Qualifications:

  • Current Georgia PT license or eligible compact license holder
  • Minimum of 2 years home health experience preferred
  • Familiarity with HomeCare Homebase software is an advantage
  • Valid drivers license for travel between assigned counties (Lowndes, Echols, Brooks, Cook, Berrien, Lanier)
  • Strong clinical assessment, documentation, and communication skills

Core Responsibilities:

  • Perform physical therapy evaluations and treatments based on physician referrals
  • Complete thorough assessments using appropriate evaluation tools
  • Review and interpret patient records, diagnoses, medical history, and contraindications
  • Develop individualized treatment plans with measurable goals to promote patient independence and function
  • Reassess and adjust treatment plans as needed for effective patient progress
  • Communicate effectively with the care team, patient, physicians, and family members regarding care plans and progress
  • Advise on adaptive equipment needs and incorporate these tools into therapy programs
  • Ensure accurate, timely documentation and compliance with regulatory requirements
  • Adhere to ethical standards, organizational policies, and federal/state laws
  • Supervise Physical Therapist Assistants and Aides during patient care
  • Engage in continuous professional development and stay current with best practices
