Schedule/Hours:

• Travel position; flexible scheduling possible depending on facility needs

Qualifications/Desired Experience:

• Graduate of an accredited Physical Therapy (PT) program
• Current licensure to practice as a PT in Indiana (or eligibility)
• BLS certification (current or willing to obtain)
• Previous experience in a hospital or home health setting preferred
• Strong communication, problem-solving, and organizational skills
• Adaptability to varied patient needs and locations

Primary Responsibilities:

• Deliver comprehensive physical therapy assessments and treatments to patients in hospital and home health environments
• Develop individualized plans of care to optimize patient mobility and recovery
• Collaborate with physicians, nursing staff, and family members to ensure continuity of care
• Accurately document patient progress and maintain up-to-date records
• Educate patients and caregivers on safe mobility strategies and home exercises
• Travel to patient homes or facilities as needed within the coverage area

$1,984 - $2,099/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
