- Monday through Friday
- No on-call requirements
Qualifications
- Active North Carolina Physical Therapist license
- Proficiency with Home Health PT, OASIS, and HCHB platforms
- Strong communication and documentation skills
- Valid drivers license for multi-county coverage (Forsyth, Davidson, Stokes, Surry)
Responsibilities
- Perform comprehensive evaluations and physical therapy treatments based on physician referrals
- Establish individualized, measurable goals and tailor plans to foster patient independence and optimal function
- Re-assess and revise treatment plans for effectiveness and goal progression
- Document all evaluations, treatments, and communications accurately, maintaining compliance with all regulatory and policy guidelines
- Recommend adaptive equipment to enhance patient care, supporting functional recovery
- Supervise Physical Therapist Assistants and Aides per state practice act
- Engage in ongoing professional development and staff meetings
