Key Responsibilities
- Perform comprehensive patient evaluations and implement therapeutic services tailored to the physicians orders.
- Maintain responsibility for supervision of Physical Therapy Assistants and Home Health Aides as assigned.
- Collaborate closely with physicians to develop and update individualized care plans.
- Conduct regular assessments, track patient progress, and provide detailed reports to physicians and the clinical team.
- Guide, educate, and supervise both family members and other agency professionals on patient therapy programs.
- Participate in continuing education and in-service training opportunities.
- Evaluate and provide recommendations for the home environment to maximize patient safety and effectiveness of therapy.
Desired Qualifications
- Successful completion of an accredited physical therapy program.
- Current, unencumbered physical therapy license for the relevant state.
- CPR certification required.
- At least 2-3 years of experience in physical therapy preferred.
- Proficiency in basic computer functions, including online HR services and training.
- Reliable transportation, valid drivers license, and required liability insurance.
