Key Responsibilities

  • Perform comprehensive patient evaluations and implement therapeutic services tailored to the physicians orders.
  • Maintain responsibility for supervision of Physical Therapy Assistants and Home Health Aides as assigned.
  • Collaborate closely with physicians to develop and update individualized care plans.
  • Conduct regular assessments, track patient progress, and provide detailed reports to physicians and the clinical team.
  • Guide, educate, and supervise both family members and other agency professionals on patient therapy programs.
  • Participate in continuing education and in-service training opportunities.
  • Evaluate and provide recommendations for the home environment to maximize patient safety and effectiveness of therapy.

Desired Qualifications

  • Successful completion of an accredited physical therapy program.
  • Current, unencumbered physical therapy license for the relevant state.
  • CPR certification required.
  • At least 2-3 years of experience in physical therapy preferred.
  • Proficiency in basic computer functions, including online HR services and training.
  • Reliable transportation, valid drivers license, and required liability insurance.
