Schedule:
- Monday to Friday, 8:00am 5:00pm
This assignment covers multiple counties (Butler, Crenshaw, Lowndes, Covington), offering you a dynamic caseload and the chance to expand your expertise across diverse communities. Key Qualifications:
- Valid Physical Therapist license required
- At least one year of Home Health experience required
- HCHB experience preferred, but not required
- Strong interpersonal, communication, and documentation skills
- Proven ability to supervise support staff effectively
Primary Responsibilities:
- Conduct physical therapy evaluations and treatments based on provider orders/referrals
- Utilize appropriate assessment tools to craft comprehensive and individualized treatment plans
- Review records to ensure safe and effective care delivery
- Set measurable goals to maximize patient function and independence
- Routinely re-assess and adjust treatment plans as needed
- Maintain clear communication with physicians, treatment teams, and families
- Recommend/adapt special equipment to enhance rehabilitation outcomes
- Complete and submit documentation promptly to maintain compliance
- Supervise PTAs and Therapy Aides within scope of practice
- Participate in staff development meetings and pursue ongoing clinical education
