Key Qualifications & Experience:
- Current and active California Physical Therapist license required
- Skilled Nursing Facility experience preferred; mentorship available for those with 0-1 year of experience
- Strong skills in documentation and time management
- Ability to work within a flexible, collaborative environment
- Comfortable with schedule options: preference for SundayThursday or TuesdaySaturday shifts
- Willingness to float, as needed, to a sister facility located within walking distance
Primary Responsibilities:
- Evaluate and treat patients within the skilled nursing facility, creating tailored treatment plans focused on improving strength, mobility, and daily function
- Collaborate with an interdisciplinary teamincluding nurses, physicians, and occupational therapiststo ensure comprehensive patient care
- Maintain up-to-date, accurate records and complete documentation using Net Health software in accordance with facility standards
- Adapt therapies as needed to support patient progress and facility expectations
