Schedule/Hours:
- Monday through Friday
- No on-call requirements
Qualifications:
- Current, active NC Physical Therapist license
- Competency with Home Health PT, Oasis, and HCHB systems
- Ability to supervise Physical Therapist Assistants and Aides
- Experience meeting productivity requirements (30 visits per week)
- Valid drivers license for coverage across Forsyth, Davidson, Stokes, and Surry counties
Responsibilities:
- Conduct comprehensive physical therapy evaluations and implement evidence-based treatment plans
- Maintain meticulous records and documentation in accordance with all regulatory standards
- Review medical records, establish measurable goals, and routinely reassess patient progress
- Communicate effectively with physicians, patients, families, and interdisciplinary teams
- Recommend and assist with acquisition of adaptive equipment to maximize rehabilitation outcomes
- Ensure adherence to professional, ethical, and regulatory guidelines
- Participate in ongoing clinical education and staff development programs
