Schedule/Hours:

  • Monday through Friday
  • No on-call requirements

Qualifications:

  • Current, active NC Physical Therapist license
  • Competency with Home Health PT, Oasis, and HCHB systems
  • Ability to supervise Physical Therapist Assistants and Aides
  • Experience meeting productivity requirements (30 visits per week)
  • Valid drivers license for coverage across Forsyth, Davidson, Stokes, and Surry counties

Responsibilities:

  • Conduct comprehensive physical therapy evaluations and implement evidence-based treatment plans
  • Maintain meticulous records and documentation in accordance with all regulatory standards
  • Review medical records, establish measurable goals, and routinely reassess patient progress
  • Communicate effectively with physicians, patients, families, and interdisciplinary teams
  • Recommend and assist with acquisition of adaptive equipment to maximize rehabilitation outcomes
  • Ensure adherence to professional, ethical, and regulatory guidelines
  • Participate in ongoing clinical education and staff development programs
